An established or returning patient is someone who has received professional services from you or another provider of the same specialty in your practice within the past 3 years.
When examining an existing patient whether for a new injury or follow-up during a continued course of care it would be appropriate to use E&M codes 99211-99215 for this evaluation.
*Note: The E&M code must be appended with modifier 25 if performed on the same day as treatment.
